Family Medicine

What are your program codes for Residency Finder and NRMP?

Our Program identification numbers are:
ACGME ID: 120010073
NRMP Code: 2335120C

What are the USMLE Score requirements?

Our USMLE Score requirements for admittance to our residency program are listed in our ERAS Program description (you can access FREIDA online). We require that you have passed the USMLE Step 1 by the second attempt and USMLE Step 2 on your first attempt. We do not require USMLE Step 3 (and the program pays for the first attempt for Matched applicants). Almost all who receive an interview most likely passed each step exam on their first attempt.

Do you require applicants to have U.S. clinical experience, externship or observership?

We prefer prior U.S. clinical experience and/or letters of recommendation from clinicians working in the U.S. or U.S-licensed physicians working abroad. Crestwood Family Medicine residency program does not offer any non-U.S. citizen IMG observerships, externships or research positions.

What else do I need to know as an IMG?

We accept U.S. MD, DOs as well as International Medical Graduates (IMGs). All applicants must participate in the NRMP match through the ERAS system. Qualified IMG applications sent prior to November 15th will receive priority review status.

What are graduation date requirements?

We require IMGs to have graduated medical school less than three (3) years ago.

Can I email the program some questions specific to my application?

Unfortunately, we are not able to respond to inquiries by email or phone. Please send all applications for Crestwood Family Medicine Residency Program via ERAS and, if we need additional information to answer your questions, we will contact you.

Do you sponsor visas?

Our program provides J1 visa sponsorship only at this time.

Is your program doing in-person or virtual interviews?

While we will mostly be doing in person interviews, we understand that not everyone is able to travel. We do not want this to be a limiting factor, so we will be offering virtual interviews to qualified applicants.

Are there any individuals who are not eligible to apply?

Graduates of medical universities excluded by the Alabama Medical Licensure Board will not be considered ( As such, these schools are ineligible for our program:

  • Universidad Tecnolgica de Santiago, Dominican Republic (UTESA)
  • Universidad Eugenio Maria de Hostos, Dominican Republic (UNIREMHOS)
  • St. Christopher’s College, Dakar, Senegal
  • University of Health Sciences Antigua, St. Johns (Antigua), Dominican Republic
